This Supervision Agreement template is drafted with the Psychology Board of Australia’s Guidelines for Supervisors as a foremost consideration. Ensure you have adequate terms of engagement in place in respect of privacy, informed consent, the risk of practice, and the storage of your supervisees information.
Included in the Supervision Agreement template are optional terms in relation to:

- Addendum 1 – Storage of Credit Card details in accordance with PCI DSS standards;
- Addendum 2 – Consent to disclose the client file to third parties (including machinery ensuring cyber security);
